Instructions to the Assessor

Explain the purpose of administering the M-SCRIM to the patient and the caregiver: to assess community reintegration following stroke in order to plan on improving or maintaining it.

Start with demographic information and state whether this is baseline or follow up assessment.

Administer the M-SCRIM by reading out the items and responses, in case of a patient not able to communicate the caregiver must be interviewed.

The M-SCRIM mostly uses 4 point Likert scale as follows:

No (0)     Able with major help (1)     Able with minor help (2)     Able with no help (3)    

Major help refers to manpower (caregiver) and mobility aid

Minor help refers to just a mobility aid.

At the end of the interview, add points for each domain and calculate the total score for the patient.

The Rural MSCRIM: Out of 95 points, multiply by 100% to get the patient score.

The Urban MSCRIM: Out of 112 points, multiply by 100% to get the patient score.

Interpret and provide feedback for the patient by placing the patient’s score on the scale below.

Cut off points:

  • 80% and above means full reintegration
  • 7960% moderate reintegration
  • 59–41% minimal reintegration
  • 40–0% no reintegration

Discuss the way forward regarding the patient’s rehabilitation goals to improve community reintegration with the patient and caregiver. Setting of rehabilitation goals should be guided by the domain(s) and item(s) that the patient obtained the lowest scores.


Instructions to the Patients

The therapist will read the items concerning your community reintegration following stroke to you (patient and/or caregiver), please respond appropriately.
At end of the interview the therapist will give, you feedback regarding your community reintegration and discuss the way forward regarding your rehabilitation.
